Deadline: Friday, May 15, 2026 at 5pm. This form is a supplement to the regional Hometown Pride application that your community is participating in. A representative from each participating town must complete this form. Your answers should address your town specifically.

Champions

Identify 1-2 individuals in your town who will commit to be a “champion” for Hometown Pride if selected for participation (this can include the person filling out this form). A champion will help initiate the program in their community by participating in the process of selecting a Hometown Pride “coach,” connecting the coach with local leaders and assisting with volunteer recruitment (Expected time commitment: 1-3 hours per month for 9 months).
